The Integrative Sleep Fellowship Experience
Sleep disorders are among the most common health conditions encountered in clinical practice, yet most clinicians receive little or no formal training in sleep medicine. The Integrative Sleep Fellowship was created to bridge that gap.
Through a structured curriculum, practical implementation, and ongoing mentorship, you'll learn how to systematically evaluate and treat the full spectrum of sleep disorders using both conventional and evidence-based integrative approaches.
Sleep disorders are rarely solved with a single treatment modality.
Medications, CBT-I, PAP therapy, and sleep studies all have an important role—but they don't address every patient or every clinical situation.
Clinicians need a broader clinical toolkit. That's why the fellowship integrates behavioral sleep medicine, circadian biology, nutrition, supplements, mind-body medicine, sleep technology, deprescribing, and thoughtful medication management when appropriate.
Unlike traditional online courses, the fellowship emphasizes implementation, not just information.
Through live case discussions, Skills Labs, AI clinical tools, clinical protocols, and ongoing faculty mentorship, you'll continually apply what you learn, strengthen your clinical judgment, and build practical skills you can use in everyday practice.
Sleep medicine is too complex to master through lectures alone. Clinical competence is built through structured learning, deliberate practice, and mentorship over time.
Our goal isn't simply to teach sleep medicine. It's to help you develop the knowledge, clinical judgment, and practical skills to provide better care for patients with sleep issues.

Why Sleep Feels Harder Than It Should in Clinical Practice
Most Clinicians Were Never Trained in Structured Insomnia Care.
Â
You’re ten minutes into an appointment when your patient asks:
“What else can I try? Nothing’s working.”
For a moment, you're not sure what to say next.
You’ve already tried the usual tools.
✓ The medications aren’t helping
✓ The supplements haven’t worked
✓ The CBT-I referral list is months long
✓ You've already worked on sleep hygiene and recommended the relaxation apps
Eventually many clinicians find themselves thinking:
“I’m not sure what to do next.”
Â
It’s not your fault.
Medical school might give you a lecture on sleep hygiene or sleep apnea and residency may include a sleep rotation if you're lucky. Therapy training might cover CBT-I basics. Even sleep fellowships focus mostly on sleep apnea.Â
But integrative insomnia care that works in everyday clinical practice is almost never taught.
So when a patient returns month after month still not sleeping, when the sleep study comes back normal but they're still exhausted, when you've tried everything you know and nothing sticks...
It's because nobody ever gave you the complete picture.
Imagine Your Practice One Year Into the Fellowship.
Â
Sleep disorders are rarely straightforward. Patients often present with multiple contributing factors—circadian disruption, hyperarousal, sleep apnea, restless legs syndrome, medications, mental health conditions, hormonal changes, or several of these at once.
Medical training often teaches individual treatments. The Integrative Sleep Fellowship teaches a clinical framework for thinking through the patient in front of you.
You'll learn where to start, what to assess, what to rule out next, and how to build thoughtful, individualized treatment plans using both conventional and evidence-based integrative approaches. And when you encounter a challenging case, you'll have mentors, colleagues, and sleep specialists to help you think it through.
Imagine walking into your next sleep consultation with a clear framework for evaluation and treatment.
Imagine knowing what to do when the first treatment doesn't work, not because you've memorized more information, but because you've learned how to think systematically.
Imagine patients returning to tell you they're finally sleeping through the night, feeling better, and getting their lives back.
That's what the Integrative Sleep Fellowship is designed to build: lasting clinical competence that transforms the care you provide for patients with sleep disorders.
Â
Why Most Sleep Education Never Changes Clinical Practice
If another online course were enough, you'd probably already feel confident treating insomnia.
You've likely attended conferences, listened to podcasts, watched webinars, and collected CME credits over the years.
Yet when a patient says,
"I've tried everything and I still can't sleep."
many clinicians still wonder:
     "What should we try next?"
That's because clinical judgment isn't built by consuming more information.
It's built by discussing real patients, seeing patterns across hundreds of cases, asking questions, making mistakes in a safe environment, and learning alongside experienced mentors.
That's why the fellowship isn't just another sleep course.
The curriculum provides the foundation. The Clinical Operating System™ helps you apply it. The expert mentorship helps you refine it.
That's how knowledge becomes clinical skill. And ultimately, that's what helps more patients sleep.
Dr. Bhopal has taught clinicians at the American Psychiatric Association Annual Meeting, Psychiatry Redefined, the Integrative Psychiatry Institute, Psych Congress Elevate, and Psychiatric Times.

How the Integrative Sleep Fellowship Works
A Structured Path to Clinical Competence
Insomnia Clarity
Â
Foundations & Assessment
Stop guessing at what's driving your patient's insomnia.
Most clinicians approach sleep complaints without a clear framework, defaulting to trial and error. This rotation gives you a structured clinical approach to identify the underlying mechanisms (circadian dysfunction, insufficient sleep drive, or hyperarousal) so every assessment leads to purposeful clinical decision-making.
What you'll master:
• Conducting structured, insomnia-focused sleep histories that reveal what's actually going on
• Using validated tools to assess severity and track meaningful progress
• Differentiating insomnia from other sleep, psychiatric, and medical conditions
• Applying a systematic framework to identify which mechanisms need to be addressed
• Understanding the sleep science that makes treatment planning logical and effective
The transformation:
You'll approach every insomnia case with clinical clarity, knowing what to assess, when to intervene, and when to refer. Your patients will feel heard and understood, and you'll have confidence in your clinical decision-making from the first appointment forward.
Sleep Testing & Evaluation
Â
Diagnostic Literacy & Interpretation
Stop feeling lost when patients hand you sleep study reports or wearable data.
Most clinicians either over-order testing or avoid it entirely because they're not confident interpreting results. This rotation builds your diagnostic literacy so you know when testing is necessary, what the findings mean, and how to translate them into clear clinical recommendations.
What you'll master:
• Determining when and what type of sleep testing is clinically indicated
• Interpreting hypnograms and sleep study reports to identify relevant findings
• Evaluating wearable sleep data with clinical discernment
• Selecting and guiding patients on appropriate laboratory screening for sleep complaints
• Integrating diagnostic findings into treatment planning and referral decisions
The transformation:
You'll confidently navigate sleep diagnostics without second-guessing yourself. When a patient brings you a sleep study or shows you their Oura Ring data, you'll know what matters, what doesn't, and how to use that information to guide your next clinical step.
Integrative Treatment Strategies
Â
Behavioral, Circadian & Evidence-Based Interventions
Stop defaulting to prescriptions because you don't know what else to offer.
Your patients are not responding to medication, but without a structured treatment approach, it's hard to know where to start. This rotation gives you evidence-based behavioral and integrative strategies that create lasting change so you can confidently offer more than another prescription or referral.
What you'll master:
• Applying core behavioral principles that address the root causes of insomnia
• Using circadian-based interventions including light therapy, timing strategies, and melatonin protocols
• Developing a streamlined, evidence-informed approach to sleep supplements
• Sequencing interventions based on what's driving the sleep disturbance
• Building personalized treatment plans that patients will follow
The transformation:
You'll have a comprehensive toolkit for treating insomnia without relying on medications alone. Your patients will see real, measurable improvement, and you'll feel confident guiding them through evidence-based interventions that create sustainable results.
Â
Complex Cases
Treatment-Resistant & Layered Presentations
Stop feeling stuck when standard approaches aren't working.
When insomnia persists despite appropriate treatment, many clinicians feel out of options. This rotation prepares you to navigate complexity, including treatment resistance, medication dependence, comorbid conditions, and presentations that don't fit the textbook.
What you'll master:
• Evaluating and managing treatment-resistant insomnia
• Developing safe, structured approaches to sleep medication tapering
• Addressing paradoxical insomnia and sleep-state misperception
• Managing sleep disturbances in hormonally complex presentations like pregnancy and perimenopause
• Supporting CPAP adherence and troubleshooting common barriers
• Sequencing interventions thoughtfully when multiple issues are layered
The transformation:
You'll approach complex cases with clinical judgment and confidence instead of frustration. When first-line strategies stall, you'll know how to troubleshoot, adjust your approach, and guide patients toward improvement, even in the most challenging presentations.
You can join at any time and start with the current rotation.
When you enroll, you get immediate access to all four rotations, the full resource library, and the community. Work through them sequentially, jump to what you need most right now, or follow along with the quarterly cohort discussions, the choice is yours.
Every rotation is reinforced through monthly live case discussion and mentorship calls — so you're never applying new frameworks alone. Bring your real patients, your hard cases, and your questions. That's where the learning becomes clinical mastery.
You can also keep the Luna AI Clinical Toolkit™ open during clinic to apply what you're learning in real time. Whether you're working through a challenging insomnia case with the Differential Diagnosis Coach or creating an evidence-based jet lag plan for a patient who's traveling, the tools are designed to fit naturally into your clinical workflow.
This isn't about watching more videos.Â
Become the clinician who walks into every sleep visit knowing exactly what to do next—with the training, support, and Clinical Operating System built for everyday practice.
A Glimpse Inside a Live Case Discussion
Each rotation is reinforced through live case discussions and faculty Q&A, so you’re applying what you learn in real time.
Here’s a short clip from one of our live sessions:Â
In this clip, we’re discussing iron testing in patients with suspected Restless Leg Syndrome (RLS), a commonly missed contributor to insomnia. This is the level of clinical reasoning and practical implementation support you receive in the program.
Guest faculty rotate throughout the year, covering topics such as women’s sleep health, complex insomnia, sleep-disordered breathing, and other conditions that often present as insomnia.

The Clinical Sleep Kit has given me the confidence to address just about every sleep concern that arises in my private practice. The flexibility of having all the material in one location allows me to customize my education on sleep medicine, and learn in real time as issues come up in my practice. With Dr. Bhopal's Clinical Sleep Kit, I have so many more tools now beyond trazodone and melatonin. Since enrolling in this program, I have a more systematic and holistic approach to working up and treating sleep concerns. The learning is dynamic, with live Q&A sessions with sleep experts, and having options to earn CE credits is a wonderful bonus. Highly recommend, 5/5 stars.

I absolutely love being a member of the Clinical Sleep Kit. Even though I can't always attend the live sessions due to time differences, when I do, they are very engaging, informative, and the atmosphere is very friendly. The replays are quickly made available, and I always learn so much from them that I can apply in my own practice as a therapist. I'm not a clinician or a prescriber however the knowledge I gain is incredibly valuable in assisting my clients/patients to discuss their sleep health with their personal physicians and advocate for the support and treatment they need. And - it's a nice bunch of like minded people!
Valerie Cacho, MD

Sleep medicine is a vast field. Even after practicing for 10 years I am still learning more and more. I love being part of Dr. Nishi's Clinical Sleep Kit community. Her monthly expert Q&As have been so valuable as I have been able to pick her brain and other sleep experts on challenging cases I have in my own practice. Another aspect of the program I enjoy is her patient handout for things like supplements and best melatonin brands. It's so organized and easy to understand!Â
Dr. Nishi has the most incredible online program in sleep medicine. If you have any interest in learning more about sleep medicine, sign up today! You'll be so glad you did. I definitely am!

Dr. Nishi Bhopal's Clinical Sleep Kit is an absolute game-changer for non-sleep medicine providers like myself. The evidence-based approach, coupled with its clear and clinically applicable information, made an immediate impact on my ability to assist patients in improving their sleep. The expert speakers featured in the program bring a wealth of knowledge to the table, enriching the learning experience. The patient handouts provided are ready-to-go and so helpful for relaying practical but high yield information. Dr. Bhopal's dedication to providing accessible, actionable insights has undoubtedly elevated the quality of care I can offer to my patients struggling with sleep issues. I highly recommend the Clinical Sleep Kit to any health care provider dealing with patients, as the importance of sleep cuts across all specialities.

About Nishi Bhopal MD
Board Certified in Psychiatry & Sleep Medicine
I didn't plan to specialize in sleep medicine.
Like many physicians, I spent years pushing through fatigue and long hours, assuming it was simply part of medical training. But my own struggles with sleep and energy led me to explore approaches that extended beyond conventional medicine, including yoga, meditation, and Ayurveda. As my sleep improved, so did my energy, my health, and my curiosity.
It also left me with an important question:
Why wasn't I learning this in my medical training?
That question shaped the rest of my career.
After completing my Sleep Medicine Fellowship at Harvard, I realized that many patients didn't fit the textbook presentations. They had overlapping sleep disorders, complex medical and psychiatric conditions, and needed more than a single treatment approach.
Over the following decade, I pursued advanced training in integrative medicine, behavioral sleep medicine, nutrition, circadian biology, and related disciplines because I wanted better answers for my patients.
That journey required years of study and more than $30,000 in additional training. The Integrative Sleep Fellowship brings those experiences together into one structured learning program, so you don't have to spend years piecing together education from multiple disciplines.Â
The Integrative Sleep Fellowship brings those experiences together into a structured learning program for clinicians. It combines the frameworks, clinical systems, mentorship, and practical tools I wish I'd had earlier in my own career.
Today, I've had the privilege of training more than 170 clinicians and teaching for organizations including the American Psychiatric Association, Psychiatry Redefined, the Integrative Psychiatry Institute, and Psych Congress Elevate.
My mission isn't simply to teach sleep medicine.
It's to help more clinicians deliver the kind of sleep care they wish they had been trained to provide, so more patients can finally get the help they need.
If that mission resonates with you, you're invited to the Integrative Sleep Fellowship.
To your sleep education,
Nishi

I am a psychiatrist who was feeling overwhelmed by the large percentage of my patients with sleep complaints, and I wanted to gain more knowledge with regard to assistance with sleep concerns that isn't dependent on sleep medications. Before this program, my only knowledge was with regard to medication information for sedative/hypnotics. I appreciate the ability to work at my own pace and access the information when I have the time to utilize it. Since starting the program, I’m able to have more discussion with patients regarding their sleep concerns and education regarding practices to improve their sleep hygiene that don't rely on sedative-hypnotics. It’s well worth the value and time.
Morgan Adams, MSW
Holistic Sleep Coach
I’m a sleep coach and wanted to have more sleep resources. At first, I was concerned that as a sleep coach this wasn’t the right platform for me. However, it’s been a great place to get insights and feedback from professionals who have more experience with certain sleep sub-topics.
Nishi is always finding great experts to give presentations, and the community is always willing to help whenever I have a question. I have more confidence that I can find the right answer for a question when I’m unsure about something. Absolutely join this program if you want to learn more about how to work with clients/patients who struggle with sleep!

I was looking for sleep resources and wanted to save time and find a reliable resource to learn more about sleep. I paid in full for a year and I love the community and I am getting a lot more for what I am paying. I have not been a part of a community like this. I love that the community is open to suggestions and I’m more confident in talking about sleep. The program has ample resources on sleep and is one of the best programs to assist with sleep resources and support.
Michelle Jonelis, MD
Neurologist, Sleep & Lifestyle Medicine Physician and founder of Lifestyle Sleep
Before this program, I’d been wanting to join a discussion community of other clinicians with similar practice styles, so that I could discuss cases and share knowledge with colleagues.
The Clinical Sleep Kit has been a great way to share knowledge and it’s helpful to have a community to discuss cases with. If you are interested in having support in managing patients with sleep disorders, this is a great place to get it!
